International

  1. Business License issued by your Country/Municipality.

Alabama

  1.  Alabama Sales Tax License.
  2.  Alabama Resale Certificate, which must include your signature.

    *If you only sell wholesale you can submit your Alabama State Sales and Use Tax Certificate of exemption AND your FEIN #.

Alaska

To qualify for a wholesale account at Nina Designs please submit a copy of your state-issued business license or other proof of established business. Alaska does not collect sales and use tax and we therefore do not requite a sales tax exemption form.

Arizona

  1. Arizona Transaction Privilege Tax License
  2. Arizona Resale Certificate Form 5000A which must include your signature.

Arkansas

  1. Arkansas State Sales and Use Tax Permit 
  2. Arkansas Sales and Use Tax Exemption Certificate, which must include your signature. 

California

  1. California Seller's Permit
  2. California Resale Certificate Form CDTFA230, which must include your signature.

Colorado

  1. Colorado Sales Tax License
  2. Sales Tax Exemption Certificate, which must include your signature.

Connecticut

  1. Connecticut Sales Tax and Use Permit
  2. Sales and Use Tax Resale Certificate, which must include your signature.

Delaware

To qualify for a wholesale account at Nina Designs please submit a copy of your Delaware state issued Business License. Delaware does not collect sales and use tax. 

Florida

  1. Florida Annual Resale Certificate

Georgia

  1. Georgia Sales Tax Certificate of Registration
  2. Sales Tax Certificate of Exemption, which must include your signature.

Hawaii

  1. Hawaii General Tax License
  2. Hawaii Resale Certificate for Goods General Form, which must include your signature.

Idaho

  1. Idaho Seller's Permit
  2. Sales Tax Resale Exemption Form ST-101, which must include your signature.

Illinois

  1. Illinoise Certificate of Registration
  2. Illinois Certificate of Resale Form CRT-61, which must include your signature.

Indiana

  1. Registered Retail Merchant Certificate
  2. Indiana Sales Tax Exemption Form ST-105, which must include your signature.

Iowa

  1. Iowa Retail Sales Tax Permit
  2. Iowa Sales Tax Exemption Certificate, which must include your signature.

Kansas

  1. Kansas Retailers' Sales Tax Registration Certificate
  2. Kansas Resale Exemption Form, which must include your signature.

Kentucky

  1. Kentucky Sales and Use Tax Permit
  2. Kentucky Resale Certificate, which must include your signature.

Louisiana

  1. Louisiana Sales Tax Registration Certificate
  2. Lousiana Resale Certificate, which must include your signature.

Maine

  1. Maine Resale Certificate
  2. Maine Certificate of Exemption, which must include your signature.

Maryland

  1. Maryland Sales & Use Tax License
  2. Maryland Resale Certificate, which must include your signature.

Massachusetts

  1. Massachusetts Sales and Use Tax Registration Registration
  2. Massachusetts Sales Tax Resale Certificate, which must include your signature.

Michigan

  1. Michigan Sales and Use Tax Certificate
  2. Michigan Sales and Use Tax Certificate of Exemption, which must include your signature.

Minnesota

  1. Minnesota Tax Identification (ID) Number
  2. MN Certificate of Exemption, which must include your signature. 

Mississippi

  1. Mississippi Retailers Permit
  2. Mississippi Certificate of Exemption, which must include your signature.

Missouri

  1. Missouri Retail Sales License
  2. Missouri Sales & Use Tax Exemption Certificate, which must include your signature.

Montana

Montana does not collect sales and use tax. To sign up for a wholesale Nina Designs account, we require a state-issued business license or other proof of established business or customer base. 

Nebraska

  1. Nebraska Sales Tax Permit
  2. Nebraska Resale Certificate, which must include your signature.

Nevada

  1. Nevada Sales Tax Permit 
  2. Nevada Certificate of Exemption, which must include your signature. 

New Hampshire

New Hampshire does not collect sales and use tax. To sign up for a wholesale Nina Designs account, we require a state-issued business license or other proof of established business or customer base. 

New Jersey

  1. New Jersey Certificate of Authority Sales and Use Tax Certificate
  2. State of NJ Resale Certificate Form ST-3, which must include your signature.

New Mexico

  1. New Mexico Registration Certificate

New York

  1. New York Certificate of Authority
  2. New York Resale Certificate, which must include your signature.

North Carolina

  1. North Carolina Sales and Use Tax
  2. North Carolina Certificate of Exemption, which must include your signature. 

North Dakota

  1. North Dakota Sales Tax & Use Permit
  2. North Carolina Certificate of Exemption, which must include your signature. 

Ohio

  1. Ohio Vendor's License or Transient Vendor's License
  2. Ohio Sales & Use Tax Exemption Certificate, which must include your signature. 

Oklahoma

  1. Oklahoma Sales Tax Permit
  2. Oklahoma Exemption Certificate, which must include your signature.

Oregon

Oregon does not collect sales and use tax. To sign up for a wholesale Nina Designs account, we require a state-issued business license or other proof of established business or customer base.

Pennsylvania

  1. Pennsylvania Sales Tax License
  2. Pennsylvania Exemption Certificate, which must include your signature.  

Rhode Island

  1. Rhode Isaland Permit to Make Sales at Retail
  2. Rhode Island Resale Certificate, which must include your signature. 

South Carolina

  1. South Carolina Retail Sales Tax License
  2. South Carolina Resale Certificate, which must include your signature.

South Dakota

  1. South Dakota Sales Tax License 
  2. South Dakota Exemption Certificate, which must include your signature.  

Tennessee

  1. Tennesse Certificate of Registration
  2. Tennesse Blanket Resale Certificate, which must include your signature.

Texas

  1. Texas Sales & Use Tax Permit
  2. Texas Resale Certificate, which must include your signature.

Utah

  1. Utah Sales Tax License and/or Use Tax Certificate of Registration
  2. Utah State Tax Commission Exemption Certificate, which must include your signature.

Vermont

  1. Vermont Sales and Use Tax Registration License 
  2. Vermont Sales Tax Exemption Certificate, which must include your signature. 

Virginia

  1. Certificate of Registration for the Collection of Virginia Sales and Use Tax
  2. Virginia Sales and Use Tax Certificate of Exemption, which must include your signature.

Washington

To qualify for a wholesale account at Nina Designs please submit a copy of your Reseller Permit and a completed Resale Exemption Certificate. Download the PDF certificate to your desktop to fill it in, then submit the signed and dated copy to us before placing your first order.  

Washington DC

  1. DC Certificate of Resale, which must include your signature.

West Virginia

  1. West Virginia Business Registration Certificate
  2. West Virginia Certificate of Exemption, which must include your signature.

Wisconsin

  1. Wisconsin Seller's Permit
  2. WI Sales & Use Tax Exemption Certificate, which must include your signature.

Wyoming

  1. Wyoming Sales & Use Tax License
  2. Wyoming SSUTA Certificate of Exemption, whih must include your signature.
